The purpose of this form is to request Office Hours with the SSIS Help Desk for technical assistance around:

  • Clearing Log
  • Intakes Needing Action
  • Unassigned Intakes
  • Inactivating Users
  • Data Integrity Reports


SSIS Mentors/Coordinators may fill this form out to request assistance within the categories above.  The Help Desk will reach out and coordinate Office Hours within 2 to 3 business days.
